The ceiling spray arm has fallen off and not sure ...

The ceiling spray arm has fallen off and not sure how to reassemble. I have a black round large washer, the actual spray arm and the centre piece to push on to the ceiling but nothing holds together. Where should the black was be, in or outside the ceiling? Manual does not explain how to do this procedure. Thanks, Deborah Baker
